Making sound decisions about your dental care and oral health is an easy thing to do with the right preparation beforehand:

• Make a list of questions to ask our office, so you don't forget anything on the day of your appointment. This includes any concerns you have, or oral problems you've been experiencing.
• If you have dental insurance, remember to bring your drivers licence, social securyty# and insurance card with you. All this information will allow us to file your claims with your insurance.

CONSERVATION OF YOUR TIME AND DOLLARS

In the interest of conserving our patient's time, we will try to complete treatment in as few visits as possible and make every effort to remain on schedule. We feel that our patient's time is valuable and you can expect us to be prompt.

We do our best to adjust to our patient's requirements in planning for their investment in optimum oral health. Therefore, we feel that it is our responsibility to inform all patients of the cost required for various treatment plans and to offer assistance in arranging payment.

INSURANCE

As a convenience to you, our office will submit charges for services rendered to your insurance carrier, but we consider the patient primarily responsible for the account. The service provided by our office amounts to an agreement between the patient and the dentist. The insurance relationship constitutes an agreement between the carrier and the patient. There is a wide variety of dental insurance coverage offered. Various programs cover from as little as 30% to as much as 80-100%. Every dental plan has a provision for limiting dollar disbursement by the insurance company for covered services. The type of services covered and the dollar amount assigned to it are determined by the type of plan purchased by your employer. Feel free to call us if you have any questions regarding your account or changes in your insurance. A simple phone call can prevent any misunderstandings.

GET ACQUAINTED

On your first visit, we will ask you for personal data and health background information. This general information is designed to help us complete insurance forms or other correspondence. This personal data is secured, both by lock and key, and when on the computer, is secured by a password, encryption and a firewall. Rest assured, your information is safe. Additionally, your information is not shared without your prior consent.

The health information provides a baseline history to aid us in providing quality dental care for you. Trying to care for a patient without a thorough knowledge of their health status, including medications they are taking, would be like trying to treat them blindfolded.

Please keep us informed of any changes in your name, address, phone number, or insurance policies; and any changes in your general health or medications.

Also, during your first visit, a thorough examination and cleaning will be performed (unless your first visit is of an emergency nature). After that we will collect information such as x-rays and other diagnostic resources. These are necessary in order to make an accurate diagnosis of the condition of your mouth, teeth, joints, gums and bone.

We will discuss our findings and answer any questions you may have about the state of your oral health. This comprehensive series of appointments will insure accurate diagnoses and give us a baseline with which to compare future conditions..
